Changes in HIV-1 Subtypes/Sub-Subtypes, and Transmitted Drug Resistance Among ART-Naïve HIV-Infected Individuals — China, 2004–2022

A new study conducted in China reveals significant alterations in the prevalence of different strains of HIV and drug resistance in treatment-naïve individuals between 2004 and 2022. The study, which analyzed sequences from the HIV-1 pol gene region, found a decline in the prevalence of certain strains and an increase in others over time. Additionally, the study highlights an increase in transmitted drug resistance (TDR) to certain drugs, particularly non-nucleoside reverse transcriptase inhibitors (NNRTIs). The overall prevalence of TDR in China was found to be at a moderate level, with NNRTI resistance standing prominently. This poses a challenge to the effectiveness of antiretroviral therapy (ART) regimens, especially among newly diagnosed individuals. Considering China's population of over 1.1 million individuals living with HIV/AIDS, urgent measures to curb TDR are necessary. This study sheds light on the need for increased baseline testing for drug resistance and better management of TDR in low- and middle-income countries.
